Directions from route 93:  Take right off exit 2; Turn right on to South Policy (at the lights) 1.1 miles; Turn right on RT 38 Lowell Rd (at the lights) 0.4 miles; Turn Left on Delaware Dr (second building on left).

Don’t forget your waiver.  Also, please wear socks and loose clothing.

WAIVER FORM

I, __________________, and on behalf of my son/daughter, __________, (hereinafter “Releasors”), in consideration for being permitted access to and entrance upon the play area and participating in any parties or programs at C & G Party Center, LLC (hereinafter “Releasees”), the undersigned, on his or her own behalf and on behalf of the minor(s) identified below, acknowledge and agree that I:

1.             Willingly agree to comply with all rules and conditions for participation and I understand and acknowledge that any participation in, or activity upon, any equipment or inflatable recreational device poses a potential for significant injury, including the potential for paralysis and even death:

2.             HEREBY RELEASE, WAIVE, DISCHARGE AND COVENANT NOT TO SUE C & G PARTY CENTER, LLC, ITS OWNERS, OFFICERS, EMPLOYEES AND AGENTS FROM ALL LIABILITY to the Releasors for any and all loss,  damage, claims or demands caused, directly or indirectly, on account of any injury to the Releasors, whether caused by the negligence of the Releasees or otherwise while the Releasors are participating, using, observing, or present in any game, practice or other event organized or initiated by the Releasees.

3.             HEREBY AGREE TO INDEMNIFY AND SAVE AND HOLD HARMLESS THE RELEASEES from any loss, liability, damage, or cost they may incur due to the Releasees’ participation or use, in any way, of the facilities, inflatable devices, or other recreational devices by the Releasors, whether caused by the negligence of the Releasees or otherwise.

4.             HEREBY ASSUMES FULL RESPONSIBILITY FOR AND ANY RISK

OF BODILY INJURY DUE TO THE NEGLIGENCE OF "RELEASEES" OR OTHERWISE while using, playing upon or participating at any time in any game, party or function within Releasors facility, whether caused by the negligence of the Releasees or otherwise.

The Releasors have read and signed the Release voluntarily.

THIS IS A RELEASE OF LIABILITY.  I HAVE READ THIS RELEASE WITH FULL KNOWLEDGE OF ITS TERMS AND CONDITIONS.
